Private Referral
- On-site safety screen to determine at-risk areas ie. kitchen, bathroom, stairs, etc.
- Equipment prescription, if required, will be facilitated through liaison with a vendor of your choice
Accessibility Assessment
- On-site visit to determine issues with home accessibility which may require modification ie. egress / ingress (enter / exit), bathroom, etc.
- Based on set criteria, application to funding sources in the community could be completed (Ontario March of Dimes, Homeowner Residential Rehabilitation Assistance Program (HRRAP)
- Some areas where we can assist with Home Accessibility include platform lifts, ramps (temporary / permanent), grab bar installation, kitchen adaptation, and bathroom modifications
- Equipment prescription / modifications, if required, will be facilitated through liaison with a contractor of your choice
Mobility assessment
- Address the need for a mobility device – ie. cane, standard walker, wheeled walker, manual wheelchair, power mobility (wheelchair and/ or scooter)
- Access to Funding through the Assistive Devices Program (ADP)
- Equipment prescription will be facilitated through liaison with a vendor of your choice
Assistive device / Adaptive aids prescription
- Address the need fir equipment to facilitate completion of personal care, housekeeping / homemaking, community access, etc.
- Equipment prescription, if required, can be facilitated through liaison with a vendor of your choice
